Some patients genuinely need more frequent care, and a small number of people with exceptional oral hygiene may be well-served by annual cleanings.

This isn't because their hygiene is poor — it's because the bacterial environment in their mouth recovers to a disease-promoting state more quickly than average. For these patients, a twice-yearly schedule allows pockets to reinfect between visits. Three-month intervals keep the bacteria load low enough to prevent reattachment of gum disease.
